Stage - I
Every participant of this quiz will receive a participation certificate with detailed analysis of his / her scientific quotient. All students with
score 7 and above will receive a special Certificate. Top ranking students will receive Diamond and Gold certificates. Top 3 students in each
grade from each school will receive Gold, Silver and Bronze Medals subject to minimum of 100 students participation in each Grade. The
name of Top 100 schools (on the basis of Total points scored by the students of that school) will be published in the Quiz Souvenir.
Stage - II
On the basis of net points accumulated by the participants, the Top 3 best Performing Schools from each district will be eligible for a Cash
Prize of Rs. 10,000/-, Rs. 5,000/- and Rs. 3,000/- respectively along with a Gold / Silver / Bronze trophy.
Stage - III
Three Schools which are adjudged the best in the State will receive Diamond, Gold and Silver trophies respectively. In addition to these
trophies the 3 best schools will also be awarded a Cash Award of Rs. 1,00,000/-, Rs. 50,000/- and Rs. 25,000/- respectively. All Schools and
participants at the State Level will receive a Participation Cup.

An Introduction on OMR Sheets


For many of you The 2012 Science Quiz might be a first interface with OMR sheet. An OMR (Optimal Mark Recognition) sheet is a Computer readable sheet and compiles the information
automatically on scan. We are providing you the information here on how to fill an OMR Sheet and we suggest students to go through it carefully as the stage 1 and stage 2 of The 2012 Science
quiz will be on an OMR Sheet.
During the examination you will be provided with a Question paper and an answer OMR sheet. First fill the useful information on the OMR sheet like your Name, Date of Birth, Mobile No, Question
paper code, Test Centre code etc. Please note that if this information is not provided or contains error, the computer will not be able to recognize your answer sheet.
1. How to fill in your name on the OMR sheet
Please note that the maximum length of the name (Including spaces) that can be filled in an OMR sheet is 32. If your name is of length more than 32, please short it in such a way that the length is
32 or less. For example the name of our beloved ex president is Dr Avul Pakir Jainulabdeen Abdul Kalam which is 38 characters in length. The same can be shortened as Dr A P J Abdul Kalam
which is now 20 Characters. This name in an OMR sheet can be filled as shown
in the adjacent sample.
2. How to fill your Date of Birth in The OMR Sheet.
You should fill your Date of Birth in DD MM YYYY format. If your Date of Birth is

2 July 2012, in DD MM YYYY format it becomes as 15 10 1931 and shown in
the adjacent sample.
3. What other information do we have to fill in the OMR Sheet?
Apart from Your name, Date of Birth, you also have to fill Your Father /
Guardians Mobile No, and Gender on the front side. On the opposite side you
need to fill Test Date, Question Paper Code, Test Centre Code and the code of
the school you are studying in. A sample of mobile no as 9840159263, Gender
as Female, Test Date as 30 08 2012, Question Paper code as 123456, Test
Centre code as 789456 and school code as 106924 is shown in the adjacent
sample.
4. Is there any other information Do I have to fill in The OMR Sheet?
Yes. Apart from This info you have to provide your Fathers Name, Occupation,
Mailing Address, Permanent Address, Email address and Tel Nos in The
examination OMR Sheet, how ever there is no need to darken the bubbles for
these values. You just need to fill in these details with a regular pen and capital
letters only.
5. How to answer the Questions in The OMR Sheet.
As shown in the sample paper, each question will have 4 options out of which
only one will be correct. Choose your correct option and darken the appropriate
bubble (A B C or D) in front of the question No.
6. What all to carry for the quiz.
Please carry multiple good quality HB pencils, Sharpener and a Good quality
eraser. Please fill all these bubbles in the OMR sheet with HB pencil only. Please
also carry a Black / Blue Ball point pen to fill your address, Email ID etc. Please
note Ink pen is not allowed.
7. What if I make a mistake in marking a bubble in the OMR sheet.
You should be extremely careful in marking the bubbles however in case a
mistake is occurred, please erase the bubble completely without damaging the
OMR sheet and mark the bubble again at an appropriate place.

Please note that all the values like Name, Codes etc are shown here for the purpose of illustration only. In the Quiz you have to fill Your Name, Your Mobile No etc and
not the value printed here. Question paper code, Test Centre Code and Your school code will be printed on the first page of your Quiz question paper.
