Note:- 1) Number of posts may change. 2) These posts are purely contractual and not state government posts. 3) Remuneration is
a consolidated pay. 4) Only short listed candidates will be eligible for interview. 5) Candidates holding higher qualification &
experience will be given preference. 6) The selected candidates will be given appointment initially for 11 months, likely to be
extended if performance is satisfactory. 7) If lesser number of applications is received, criteria for selection may be relaxed.

Commissioner (Family Welfare) & Director , National Health Mission. Arogya Bhavan, 3rd Floor, St. Georges
Hospital Compound, P.D.Mello Road, Mumbai 400 001. As per the date given into the column No. 8 along with
original certificates of Qualification required and record of experience. One set attested photo copy should also be submitted along
with application. Application should be submitted on plain paper indicating (a) Name of post applied for (b) Full Name in block
letters (c) Communication Address with Phone/Mobile No.(compulsory) & E- mail ID (d) Date of Birth as on SSC record (e)
Educational qualifications mentioning name of the course, institution and university, year of passing out and % of marks obtained (f)
Work Experience clearly mentioning organization worked for, post held, duration and responsibility of post and salary and any other
relevant details along with one passport size photo. (g) Candidates come after 11.30 a.m. will not permitted the interview.
