First Day Forms

Parent/Student Return/Information Items
Guidelines:
All forms are required for all students: returning and new to district.
REQUIRED PAPER FORMS : Forms in Section I must be completed and returned to school no later than Aug 28.
ONLINE COMPLETION OF FORMS: Forms In Section II:
o Beginning Aug 24 through Sept 15, parents may log into Skyward Family Access to complete the Online Student
Information Forms for the 2015-16 school year including the verification of demographic information, emergency and
medical information, as well as other new year required forms.
o Parents that do not have Skyward Family Access may request it from their students home campus.
o Students whose online updates are not completed by September 15 will be given the necessary forms (hard copy) to
take home for parent completion on September 18. These forms must be returned by September 30.
INFORMATION ITEMS: Forms in Sections III are for your information regarding truancy, free and reduced lunch program,
and instructions for pre-payment of meals using the computer system in the cafeteria.

PAPER FORM RETURN ITEMS: Please complete and sign the information on the following forms and have

your student return the completed forms to their 3rd period teacher by Friday, August 28:

Student Media Release Form: Review, complete, and sign the requested information on the document.
Military Connected Student Form: All parents must complete and sign the form, and one must be on file at
school for every student.

III. INFORMATION ITEMS

Free/Reduced Meal Household Letter: Completion is optional. This form may be completed at school, or you may
complete and submit the form via the following website: http://fortbendisd.heartlandapps.com.
Point of Sale Computer System Letter: Provides information for prepayment for meals as well as instructions for
inputting student ID # at register.
FBISD Student Online Information Updates Flyer: Instructions for gaining online access for student records
FBISD Truancy Information Letter: Information regarding attendance reporting procedures in FBISD.

THE ITEMS FROM SECTION I MUST BE REVIEWED, SIGNED, AND RETURNED TO SCHOOL
BY FRIDAY, AUGUST 28. Students will return these forms to their 3rd period teacher.
