PROCEDURE : ASSIST IN RECTAL

EXAMINATION
PURPOSE

Examine rectum for any abnormalities e.g : neoplasm,


haemmorrhoids, polyps, fistula, fissure or faecal impaction.
Aid in diagnosis of acute appendicitis, enlarged prostate and
pelvic disorders.

REQUIREMENT

1) Disposable gloves.
2) Lubricant.
3) Protective sheet.
4) Tissue paper / gauze.
5) Torch light.
6) Equipments – proctoscope, forceps e.g : sponge holding forcep,
probe – optional.
7) Specimen container – if needed.
8) Clinical waste.
9) Domestic waste.

STEPS

1. Verify Dr’s order.


2. Greet patient and explain procedure.
3. Prepare patient – empty bladder, position in left lateral and place
protective sheet.
4. Assist Dr in procedure – provide equipments, lubricate and collect
specimen if any (digital used proctoscope, manual used the
fingers).
5. Advice patient to take deep breath during procedure.
6. Clean patient’s perineum post procedure.
7. Make patient comfortable.
8. Label and send specimen to lab as soon as possible if any.
9. Clear and clean equipments.
10.Obverse for any complications – bleeding, pain, trauma.
11.Record and report result.

***If haemorrhage, Dr can do rubber bandage.

NURSING RESPONSIBILITIES

1. Reassure client to gain cooperation.


2. Assist procedure in a correct manner to prevent complication –
bleeding and pain.
3. Ensure client’s comfort is maintained throughout procedure.
– Left lateral position and pain free.
1. Send labeled specimens in appropriate container with lab request
form.
2. Record findings and report abnormalities.
