PATHOPHYSIOLOGY
Genetic changes. Many fibroids contain changes in genes that differ from those
in normal uterine muscle cells. There's also some evidence that fibroids run in
families and that identical twins are more likely to both have fibroids than
nonidentical twins.
Race. Black women are more likely to have fibroids than women of other racial
groups. In addition, black women have fibroids at younger ages, and they're also
likely to have more or larger fibroids.
Age: Women in pre-menopause -- typically between the ages of 45 and 50 -more commonly experience uterine fibroids because estrogen levels over time
contribute to uterine fibroids.
Other factors. Onset of menstruation at an early age, having a diet higher in red
meat and lower in green vegetables and fruit, and drinking alcohol, including
beer, appear to increase your risk of developing fibroids.
Women with fibroids may have an increased amount of menstrual bleeding, although
many women with fibroids do not have this problem. At the time of the menstrual period,
when the uterine lining is shed, the inside of the uterus is raw and bleeding. However,
because the uterus is a muscle, it also has the unique ability to contract and squeeze
the bleeding vessels of the uterus, much like stepping on a hose. These contractions
are what you may feel as menstrual cramps. The fibroids dont allow the uterus to
squeeze down properly, so the blood vessels in the uterus stay open longer, and you
lose more blood. Also, fibroids produce proteins that make blood vessels grow nearby
and other proteins that make the blood vessels expand, so that more blood is lost after
menstrual bleeding starts.
When heavy menstrual bleeding persists over time, your body may not be able to make
new blood cells fast enough to replace those that have been lost.
Iron is very important in maintaining many body functions, including the production of
hemoglobin, the molecule in your blood that carries oxygen. Iron is also necessary to
maintain healthy cells, skin, hair, and nails.
Iron from the food you eat is absorbed into the body by the cells that line the
gastrointestinal tract; the body only absorbs a small fraction of the iron you ingest. The
iron is then released into the blood stream, where a protein called transferrin attaches to
it and delivers the iron to the liver. Iron is stored in the liver as ferritin and released as
needed to make new red blood cells in the bone marrow. When red blood cells are no
longer able to function (after about 120 days in circulation), they are re-absorbed by the
spleen. Iron from these old cells can also be recycled by the body.
Anemia is often first shown by routine blood tests, which generally include a complete
blood count (CBC) which is performed by an instrument which gives an output as a
series of index numbers. A sufficiently low hemoglobin (Hb) by definition makes the
diagnosis of anemia, and a low hematocrit value is also characteristic of anemia.
Further studies will be undertaken to determine the anemia's cause. If the anemia is due
to iron deficiency, one of the first abnormal values to be noted on a CBC, as the body's
iron stores begin to be depleted, will be a high red blood cell distribution width (RDW),
reflecting an increased variability in the size of red blood cells (RBCs). In the course of
slowly depleted iron status, an increasing RDW normally appears even before anemia
appears.
A low mean corpuscular volume (MCV) often appears next during the course of body
iron depletion. It corresponds to a high number of abnormally small red blood cells. A
low MCV, a low mean corpuscular hemoglobin and/or mean corpuscular hemoglobin
concentration, and the appearance of the RBCs on visual examination of a peripheral
blood smear narrows the problem to a microcytic anemia (literally, a "small red blood
cell" anemia). The numerical values for these measures are all calculated by modern
laboratory equipment.
The blood smear of a patient with iron deficiency shows many hypochromic (pale and
relatively colorless) and rather small RBCs, and may also show poikilocytosis (variation
in shape) and anisocytosis (variation in size). With more severe iron-deficiency anemia,
the peripheral blood smear may show target cells, hypochromic pencil-shaped cells,
and occasionally small numbers of nucleated red blood cells. [15] Very commonly, the
platelet count is slightly above the high limit of normal in iron deficiency anemia (this is
mild thrombocytosis). This effect was classically postulated to be due to high
erythropoietin levels in the body as a result of anemia, cross-reacting to activate
thrombopoietin receptors in the precursor cells that make platelets; however, this
process has not been corroborated. Such slightly increased platelet counts present no
danger, but remain valuable as an indicator even if their origin is not yet known.
