I am a graduate student under the direction of Dr. Daniel Choi at California State University,
Fullerton.
I am conducting a study to improve the use of formative assessments in the subject of math at
Excelsior Charter Schools. Through personal experiences, observations, and diagnostic survey
results, I have noticed that the current paper quizzes being used in the student packets are not
serving their purpose as formative assessments. Not all students are having their quizzes
corrected, and even fewer seem to actually received feedback regarding their mistakes. My
hopes in this study is to see if the use of the online learning management system, Canvas, can
assist with providing the kind of feedback expected from effective formative assessments.
Your participation will involve granting permission for use online quizzes through Canvas in
place of the paper quizzes in the existing curriculum packets. You will also be granting me
permission to collect student quiz and test scores and to survey students each unit in order to
determine growth and success during this action research project.
In the chance that results of this study are published, no names or identifying information will
be included. Results of this study will be reported in a case study format. Research records will
be kept confidential to the extent allowed by law. Data will be kept in my Google Drive account
provided by Cal State Fullerton, to which only I have access.
Your participation is voluntary and you are free to withdraw from participation at any time
without suffering penalty or loss of benefits or services you may otherwise be entitled to.
