Declaration
(Please read carefully)

a.

Plant route plans have been obtained from Singtel

b.

Site survey / mapping done by TCDW for Singtels plant route relative to proposed
trial holes and earthworks position.

c.

Cable locations / route were marked out at site by TCDW.

d.

My staff and I have been briefed by TCDW on cable damage prevention.

e.

I have briefed the supervisor(s), workers and excavator operators on measures to

be taken to prevent damage to cable.

f.

I will stop all earthworks if telecommunication cables are damaged and immediately
inform Outside Plant Operations, Singtel.

I hereby declare that I know and understand my duties under the provision of the Telecommunication Act 1999 (and any
subsequent amendments thereto), and the information in this notice and any attached documents are true, correct and
complete to the best of my knowledge.
During the duration of the earthwork, I shall make myself readily contactable at the pager number given in this notice.

Fees
A fee of $128.40 (every 1.5km) will be charged for each Notice For Commencement of Earthworks processed by
Singtel.

Please attach the following records together with your notice:

(i)

A sketch showing your proposed site plan with proposed trial holes and earthworks position relative to Singtels
plant route.

(ii) The work schedule of your project

(iii) The Cable Detection Drawing (from your TCDW).
