Behavior Contract

This is my Behavior Contract. I was given at least two warnings before this was
given to me. Whenever I am disruptive or inattentive, I color in a bar in the
graph. Then I write a sentence or two about what I will do differently. If I color in
two bars, I will have a Time Out in the office. If I color in all three bars in one
day, then I will have a detention. Please read, sign, and return this to school
tomorrow.
