Transition Checklist for Riverside Brookfield Students

14 to 16 years of age

Participate in Next Steps Training (parents can attend this 1 time or as many times as
they would like)
Begin exploring career interests with your teen and affirm commitment to work or
commitment to day program
Request an intake meeting for DRS (Department of Rehabilitation Services) if requiring
services as determined by team.

Plan for vocational experiences during the IEP

Consider summer work options when the student is going to be 16 years of age.

Complete the Prioritization of Urgency of Need for Services (PUNS) by contacting


Suburban Access Inc. at (708) 799-9190 if your child has a developmental disability or
multiple disabilities (UPDATE PUNS EVERY YEAR SO YOUR NAME STAYS UP ON
THE LIST)
Talk with a financial planner regarding income assets in your childs name. (This is the
time to look into a Special Needs Trust)
Acquire state ID for your child

17 Years of Age

Tour adult service agencies for individuals interested in day programs or residential
programs
Investigate power of attorney and guardianship options (There are a couple options
when looking into guardianship, if you have questions or referrals contact Ronnie Carey,
Melissa Hammer, or Nicki Anderson)

Update Puns

Consider transportation options

Establish graduation date during IEP

18 Years of Age

Complete graduation credit check during IEP if student is earning a High School Diploma

Apply for SSI (Supplemental Security Income)

Apply for Medicaid

Update PUNS

Register for Selective Service (males only)

Register to Vote

Establish legal power of attorney or guardianship if necessary

Explore that there is a plan for medical/health coverage for your student

Discuss the need for additional transition services and/or community connections for
post secondary training/training, employment and independent living

18+ years to Final Year

Update PUNS to indicate exit from school

Apply to adult service agencies

Have a DRS exit planning meeting (November)

Request that community service providers be invited to IEP (this could be people from
Community Support Services, or from Suburban Access, DRS caseworker, current
employer, and so forth).
