AFFIDAVIT
I, JOCELYN MARANAN-CUEVAS, of legal age, married, Filipino and a
resident of San Pioquinto, Malvar, Batangas, after having been duly sworn to in
accordance with law hereby depose and say:
That I graduated from Lyceum of Batangas, Batangas City with
the degree of Bachelor of Science in Information and Computer
Science last April 16, 1997;
That I was issued my Diploma after graduation on that same
year;
That I discovered that the said Diploma which I have been
keeping in my cabinet was missing and despite diligent efforts to
locate the same could no longer be found;
That I am executing this affidavit to attest the truth of the
foregoing statements and to request the proper authorities concerned
to issue me a new Diploma in lieu of the lost one.
IN WITNESS WHEREOF, I have hereunto set my hand this5th day of
October, 2007 at Batangas City.
