Investigation Report

on the facts and circumstances surrounding the loss

Facts:
On or about 3:00 AM in dawn on 2nd day of July 2015 a fire broke out at Marcos Type
Building of Siocon Central School, Poblacion Siocon, Zamboanga del Norte, wherein, rapidly
spread to the adjacent offices, class rooms, school canteen and school clinic.
The investigation report of Bureau of Fire Protection (BFP) revealed that the fire was
originated at the school library and ALS room. It was due to the frequent power interruption
(brown-out/black-out) which suspected to be the cause of electrical short circuit and at the same
time due to old installation of electrical wiring. Further, they learned that the school use substandard electrical wires for electrical wiring installation and also most of the rooms were newly
painted during the brigade eskwela 2015 by the Department of Education.
Records showed that the fire had totally damaged the Marcos type building which consist
of Eight (8) Classrooms, One (1) District Supervisors Office, One (1) Principals Office, One (1)
e-Classroom, One (1) Computer Room, One (1) School Library, One (1) ALS Room, One (1)
Clinic and One (1) Canteen including property, fixture and equipments which was estimated by
the investigator in the amount of Php 5,000,000.00 more or less.
Findings/Observation:
Four (4) days has lapse the audit team responded on the place of incident. We inspected
the buildings and their properties thereto and found out that it was totally destroyed. Per ocular
inspection on the place of incident, we are on the opinion to rely on the result of investigation
conducted by BFP, that indeed the cause of fire incident was accidental and due to old
installation of electrical wirings. Considering that the latter was an expert on determining the
facts and circumstances that causes such tragic event. Likewise, they are the one who first to
respond on the alleged fire incident.
Conclusion:
Wherefore, based on foregoing premises and on the facts and informations we had
gathered, we are on the opinion to recommend the favorable merit on the request of relief from
accountability by the herein principal of Siocon Central School, Ms. Rubilita M. Tolentino.
Likewise, we relayed on the report of BFP in terms of the cause of fire incident was accidental
and due to old installation of electrical wirings.
