ER nurse note by DG nurse student at AGM Orlando, FL

-30 y/o Hispanic female came to the ER and stated that he cut his finger while chopping onions,
and cutting meat. Patient stated that he applied pressure with paper, his not feeling any pain and
his friend took him to the ER department.

-Hispanic female is A & O x3. VS: B/P 120/80, P 85, RR 18, T 98.6. Right 2 nd digit: 6 cm
laceration to distal dorsal aspect, not actively bleeding. No foreign body appreciated on visual
inspection. Sensation intact: N/V intact. No tendon involvement. Complaining of severe pain,
level of pain scale 9/10, Skin laceration on right 2 nd digit and swollen. No significant
medical/surgical Hx. Tetanus is up to date. No medications. Nonsmoker, no drugs, lives with
boyfriend.

-Actual / Risk for impaired tissue integrity related to destruction of skin layers.
This seems a deep-long laceration distal and stiches will be given to help the skin easily come
together.
-There were no complications. Patient advised to f/u with us 7-14 days to remove the stiches or
to f/u with family physician. To return to the ED at once if any signs of infection: pus, fever,
swelling, increased pain, or any concerns. To take Tylenol or Ibuprofen for any discomfort.
