the risks if I get hurt or killed during the event(s) and I state the following.

and if, at any time I feel anything to be unsafe, I will immediately leave and refuse to participate further in the event(s).
I understand that the ACTIVITIES OCCURING ON THE PREMISES AND OF THE EVENT ARE VERY DANGEROUS and INVOLVE
RISK AND DANGERS OF MY BEING SERIOUSLY INJURED OR HURT, MY BEING PARALYZED OR KILLED.
I know that these Risks and dangers may be caused by my own actions or inactions, the actions or inactions of others
participating in any activity on the premises including any of the event(s), the rules of the event(s), the condition and layout
of the premises and equipment, or negligence of others, including those persons or entities or their affiliates and
subsidiaries responsible for conducting the event(s).

Instructions & Notes

1.) Fill out all information accurately: Name of Event; Event Dates; Year; Sign and Print parent/guardians full name & Minor who will be Attending;
Fill in the Age of the Minor attending; Date.
2.) All Parents must send a copy/scan print out (both sides) of their State I.D./Drivers License, and/or any other Federal or State I.D. that has a
picture, signature, name, and address on it. This insures that minors are not signing these documents themselves, and parents are actually
consenting to the minors participation. Information like your I.D. number, or other personal information you do not want to share can be blacked
out of the photo if you wish. These print outs need to be attached to the back of this Minors Assumption of Risk Acknowledgement. Minors
arriving without a parents ID information will not be allowed into any Event. Minor will also need their I.D. if they are arriving without a parent.
3.) Minors will have a Release and Waiver of Liability and Hold Harmless Agreement, once they arrive on the premises. A copy of that document
should be filled out by parents in addition to this Acknowledgement, even though you will not be participating in the event(s). Filling the
release/waiver out, and signing, is an acknowledgement of what your minor child will be signing themselves, and the risks involved with
participation in any event(s) held by Wrights Event Center LLC, Wrights Off-Road Mud Park, and/or Mike Wright Construction. It absolves the
event holder(s) of any responsibilities from accidents/actions/inactions resulting in any bodily harm or equipment harm/failure caused by your
minor child, another participant in any event(s), or any other persons/equipment/structures/infrastructures on the premises.
4.) Sign all acknowledgements at the bottom of this page stating that you have read and understand all instructions.
5.) No minor under the age of 16 will be permitted into the park without a parent/guardian accompanying the minor. No Exceptions. Any other
adult besides the parents and/or legal guardians cannot sign legal documents for a minor. These are dangerous events, and waivers must be
filled out accurately. This rule is not up for debate once you get to the park. If another person in your family will be accompanying your minor child,
these documents will be requested upon entry. If they do you not have them, the minor will not be allowed to enter the premises.
6.) Any questions you may have can be answered via our Business Facebook page, the Event page posted for specific event, or by calling Mike
Wright (phone number at the top of the page).
I have read all instructions & notes in this document, understand them, and give consent for my minor child to participate in the event(s) held
by Wrights Event Center LLC & Wrights Off-Road Mud Park.
