JOINT AFFIDAVIT
We, ASUNCION A. BANDOLA and ARLAN E. BANDOLA both of
legal age, Filipino citizens, married, and residents of Tagaytay, Gubat,
Sorsogon, after being duly sworn according to law, do hereby depose and
state:
1. That we are the biological parents of I-AN ARIMADO BANDOLA who
was born on May 4, 2006 at Sorsogon City;
2. That on May 11, 2007, I-AN ARIMADO BANDOLA was baptized at the
chapel located at Brgy. Tagaytay, Gubat, Sorsogon;
3. That however, when we checked for the record of his baptism at our
local parish, we were informed that no record of his baptism exists;
4. That this affidavit is being executed to attest to the truth that IAN A.
BANDOLA was baptized on May 4, 2006 at Tagaytay, Gubat,
Sorsogon.
IN WITNESS WHEREOF, we have hereunto set our hands this 10 th day
