WE, LETICIA MAGTUBA BAGATUA and ALBINA EGUIA MORENO, both of

legal ages, both married, and residents of Dapitan City, after having been duly sworn to
in accordance with law, hereby depose and say:
That we personally know LUCENA M. FABIAN because we are family friends
and we have known her for more than 50 years;
That we also personally know that she has been using the name LUCENA M.
FABIAN since she was a child;
That we personally came to know from available records; her SSS Records
indicates that her name is LUCENA D. FABIAN;
That we attest that LUCENA D. FABIAN and LUCENA M. FABIAN refers to
one and the same person;
That we are executing this affidavit in order to attest to the truth of the facts
aforementioned, and for any legal purpose this may serve.
