Gastroesophageal Reflux Disease (GERD)

ABSTRAK
Penyakit refluks gastroesofageal (gastroesophageal reflux disease, GERD) kurang umum dijumpai dan derajat keparahan
endoskopiknya lebih ringan di Asia dibandingkan di negara-negara Barat. Namun, data saat ini menunjukkan bahwa telah
terjadi peningkatan frekuensi penyakit tersebut di Asia. Pemeriksaan baku emas untuk diagnosis GERD erosif adalah
endoskopi saluran cerna atas. Sementara itu, tidak terdapat pemeriksaan baku emas untuk diagnosis penyakit refluks nonerosif (non-erosive reflux disease, NERD) dan diagnosisnya mengandalkan gejala atau respons terhadap pengobatan proton
pump inhibitor (PPI). Sasaran pengobatan GERD adalah menyembuhkan esofagitis, memperingan gejala, mempertahankan
pasien tetap bebas gejala, memperbaiki kualitas hidup, dan mencegah komplikasi. Hingga saat ini, PPI merupakan terapi
medikamentosa yang paling efektif. Sesudah pengobatan awal, terapi on-demand dapat efektif pada beberapa pasien
penderita NERD atau esofagitis erosif ringan. Bedah anti-refluks oleh dokter bedah yang kompeten dapat membuahkan
hasil-akhir yang sama, dengan mortalitas operatif sebesar 0,1 0,8%. Keputusan bergantung pada pilihan pasien dan
ketersediaan dokter bedah yang berpengalaman. Pada penderita GERD yang tidak mengeluhkan gejala peringatan (alarm
symptoms) saat pemeriksaan di layanan primer, pengobatan dapat dimulai dengan PPI dosis standar selama 2 minggu. Bila
responsnya sesuai, PPI dilanjutkan selama 4 minggu sebelum masuk ke terapi on-demand.

Pendahuluan
Berdasarkan data epidemiologis, prevalensi
GERD di Asia sekitar 2-5% dan esofagitis
endoskopik sebesar 2-5%, lebih rendah
dibandingkan prevalensi di negara-negara
Barat.1-3 Derajat keparahan GERD di AsiaPasifik cenderung lebih ringan, dan secara
endoskopik normal (non-erosive reflux
disease, NERD); kalaupun didapatkan
gambaran esofagitis, sebagian besar kasus
(90%) merupakan esofagitis Los Angeles (LA)
grade A atau B.3 Esofagus Barrett, striktur
esofagus, atau adenokarsinoma esofagus
juga lebih jarang ditemukan pada pasien di
Asia dibandingkan dengan pasien di negara
Barat. Sebaliknya, prevalensi infeksi Helicobacter pylori di Asia (30-60%) lebih tinggi
dibandingkan di negara Barat.
GERD harus dibedakan dari penyakit saluran
cerna atas yang terkait H. pylori, terutama
ulkus peptikum dan kanker lambung.
Definisi
Berdasarkan Genval Workshop, definisi pasien GERD adalah semua individu yang
terpapar risiko komplikasi fisik akibat refluks
gastroesofageal, atau mereka yang mengalami gangguan nyata terkait dengan kesehatan (kualitas hidup) akibat gejala-gejala
yang terkait dengan refluks. Secara sederhana, definisi GERD adalah gangguan beru-
