PERFORMANCE EVALUATION FORM

Mr. Mahmood Ali Shah S/O Mir Sher Ali Shah is known to me from last 5 years. He is
fit & fine for any kind of job in UN, WFP and other Organization. He can perform any
kind of work like Computer Operator, Admin Work, Clerical Work, Accounting,
Distribution of Food, Communication work, operating of any kind of machine,
attending meeting.

He is except of field work. He done his duties as a Field Worker in Polio, Union
Council Communication Support Officer, Social Mobilizer, Data Operator and
Surveyor.
