Attached are embassy forms for the Express Entry Program.

The forms are as follow:

1. Generic Application Form
2. Schedule A Form
3. Supplementary Information Form
4. Schedule 3 Form
5. Additional Family Information Form
6. Additional Dependants/Declaration Form
Please print all the forms and fill them out. You can use any colored ballpen or pencil in filling
out the forms. If you are unsure on what to write just leave it blank. Those forms are not yet
final, an Analyst will finalize your forms before lodging.
If you have any questions or concerns about the embassy forms or about your application you
can contact us from Monday to Friday, 9am-12noon and 1pm-6pm. On Saturdays, 9am-12noon
and 1pm-3pm. Any inquiries done after office hours will be entertained the next day.
