AFFIDAVIT OF COHABITATION
WE, AMOR SHEILA D. ANGOT and JESUS I. APOLONIO, JR., both
of legal ages, Filipinos, and residents of Davao City, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and say:
That we have been living together as husband and wife since 2009 up
to the present without the benefit of lawful marriage;
That knowing that we are both legally capacitated to enter into a valid
marriage; we desire to legalize our union under the provision of Art. 34 of
the New Family Code of the Philippines, in relation to Art. 76 of the New Civil
Code of the Philippines;
That we are executing this affidavit to attest to the truth of the
foregoing facts in support with our immediate marriage and for whatever
legal purpose this may serve.
