h TTK Health Services as TPA, from my employer (Oracle) including dependents. Fo
r the FY 2008-09, our father Hari Gopal Sanka hospitalized (had eye surgery) at
Hyderabad (Andhra Pradesh) location and used current corporate health insurance
(with below/attached mentioned details). We need copies of health records
(including discharge summary) along with final insurance disbursement letter wit
h the ATTESTATION OF RESPECTIVE AUTHORITY. Please send all or as many as possibl
e documents related to the claims (claim done during January, 2008 around).
