MEMORANDUM OF AGREEMENT

1.

The ______________ department of _________________ (name of company) will

permit the undersigned student-trainee to undergo his/her practicum in the company
for the purpose of gaining practical knowledge and experience of the culture of the
company;

2.

The training program is designed to last for a minimum of 100 hours of working
experience. The same may however be extended by means of supplementary
agreement of the training company and the school;

3.

The status of the student-trainee while under this training shall be that of the On-TheJob Trainee, and not as the employer-employee relationship;

4.

The student-trainee agrees to observe the rules and regulations of the training
company and to abide by all the implied and stated terms provided in this agreement;

5.

The student-trainee further agrees to make every effort to attend regularly to his/her
work and to notify the training company and OJT Coordinator of any unavoidable
absence from the training;

6.

The training company agrees to make a report / evaluation to the OJT Coordinator of
work attitude, performance, attendance, and other aspect of the job of the trainee; and

7.

The Waiver of Claim hereto attached becomes part of the training agreement.

IN WITNESS WHEREOF, the parties have hereunto affixed their signature on the ____ day
