AFFIDAVIT OF DISCREPANCY
I, Remy S. Pealber, of legal age, married, Filipino citizen and presently a resident of
35 Peace Street, Multinational Village, Paraaque City, after having been duly sworn to in
accordance with law, depose and state:
1. That my name is Remy Salcedo Pealber, as stated and registered in my birth
certificate issued by the National Statistics Office, a copy of which is hereto
attached as ANNEX A;
2. That I have been using the name Remy S. Pealber in all my personal records,
documents, transactions and communications;
3. That the name Remy S. Penalber and Remy S. Pealber refer to one and the
same person, the herein Affiant;
4. That I am executing this Affidavit to clear any discrepancy which may arise from
official documents bearing either name Remy S. Penalber or Remy S. Pealber,
to attest to the truth of the foregoing facts and to use the same for whatever
legal purpose it may serve.
