Dennis Ogg came to Records on 10/07/16 to report he received a phone call from
Amsher Collection stating in February 2016 that Dish Network set up service in
Athens GA. Victim lives in Evansville IN, works in Warrick County, and has
never lived nor gave anyone permission to install Dish Network in Athens GA.
Dish Network would not give Dennis the name or address in Athens GA where
service was installed. Dennis provided Records with an Identity Theft Victim`s
Complaint & Affidavit that he filled out on line from Federal Trade Commission
(FTC). Dennis has no paperwork to prove this actually occurred in Athens GA.
Dennis has only telephone calls. See attached form from FTC.
Dennis advised he will be contacted by Financial Crime Unit 812-436-7991.

I was dispatched to assist animal control in reference to a dog in a vehicle. The


vehicle was said to not be running and the windows were not cracked. I arrived
and Animal Control Ofc K. Yarde and I located the owner of the dog and the
owner of the vehicle. During the investigation I found the owner of the vehicle to
be in possession of a handgun without a license.

Michelle came to EPD Records on 10/07/2016 to make a report of intimidation by
offender. Michelle & offender have been married for two years in August 2016.
On Wednesday, 10/05/2016, offender threatened to tie her up and take her to
N----- town and leave her there to be raped. Not to kill her but enough to teach her
a lesson. Offender also stated he is unstable and cruel. Offender said he would
get a knife to scare victim. The Behavioral Health Clinic suggested that Michelle
leave her current residence. Offender does not know where Michelle is staying.
See "ATTACHMENT" for copy of Protective Order issued out of Illinois.
Michelle was advised to get an Eviction on having the offender removed from
property that she owned prior to her marriage. Offender has PTSD and is off his
medication and has a collection of firearms in the home. Victim advised to get
eviction started on offender.

Victim states that he went to pick up the offender. When the victim got to the
house that the offender was at the offender refused to go. The victim states that he
left and went back to listed address.
The offender then came back to the listed address and entered the victims room,
and started destroying the victims room.
The offender then started pushing the victim around, and the victim states that he
tried to get the offender to calm down.
The victim states that he then tried to hold the offender back and the offender
started swinging at him.
The victim then held down the offender thinking he would finally calm down, but
offender did not.
The offender got up and the victim stated that he tried to leave. Victim states he
was trying to leave, he was walking down the stairs, (still inside the building) and
the offender came out from the apartment and jumped on his back while he was on
the stairs. Offender put him in a head lock, to which the victim talked to him and
convinced him to let him go.
Offender then went outside. He turned around and was going to go back into the
apartment, but victim had already entered the apartment and locked the door so
that he could not come back in and do more damage. Officers were called to the
scene.

On listed date the listed offender came in and removed listed product from the
shelf, took it to the front of the store and ask to exchange for a different color.
There was no different color, so the offender left with the listed item, passing all
points of sale, and walked out the door with the item.

An officer spoke with the victim of 323 S Bedford Ave in reference to her dog
being taken. The victim stated she got into an argument with the suspect about the
dog defecating inside the home. The victim stated the suspect told her that if the
dog has another accident in the home, the victim would never see her dog again.
The victim stated the suspect told her that he was going to take the dog to an open
field and let it go. The victim stated she locked her dog up in her mother`s room to
keep the suspect from getting to it. The suspect broke into the room and took the
dog. The victim stated she arrived home and the suspect got in his vehicle and fled
the scene quickly. The victim stated she checked the home for the dog
immediately and could not find it. The victim stated she tried contacting the
suspect but the suspect would not tell her where her dog was. The victim stated
the suspect has previously thrown the dog down a flight of stairs. The victim
stated she never called police because she did not think that police would do
anything. The victim stated she is afraid that she will never see her dog again and
unknown if the suspect harmed the dog or not.

Officers were dispatched to the intersection of Division St./Read St. in reference to


an investigation. Upon arrival, officers made contact with offender, Adam Hartley,
who had a misdemeanor warrant. During search incident to arrest, Hartley was in
possession of synthetic cannabinoids. Hartley was taken into custody on warrant,
transported to VCCC, and booked in on possession of synthetic cannabinoids.

not. Suspect was possibly living in a yellow home near the victim`s address on
Cherry St. Victim does have the little girls phone number that called them. Victim
stated the dog was a small 20lb white shih tzu with a blue polka dot collar, having
a dog tag with the victim `s number and name on back. Victim stated the dog has
short hair with small bumps that can`t be seen but felt.

Vanderburgh County Sheriff`s Office (VCSO) Deputies were dispatched in
reference to a theft from a motor vehicle.
Victim reported a firearm was stolen from a lock box located on the back of his
pickup truck sometime between late July 2016 and late August 2016, most likely
while at his rental property, located at 14 W Eichel Street.
The firearm had already been recovered during an un-related EPD arrest on
08/04/2016, before the victim knew the firearm had been stolen.

On 10/07/2016, at approximately 1950 hundred hours, Officers ran the plate of a


white four door passenger car in front of them near the intersection of Riverside
Drive and Lodge Avenue. The plate returned to a white 2003 Ford. Officers
clearly observed the vehicle inf front of them was not a Ford but a Buick.
A traffic stop was conducted and the driver was found to be HTV for life

Van turned right on a red light on Maryland to go North on St. Joseph. There is
a sign there that says you cannot turn right on red. When we stopped the vehicle,
we detected the odor of marijuana. Both the driver and the back seat passenger
said they had marijuana in the vehicle. There was a bong behind the driver`s seat
that the driver said was his. All people in the car were juveniles and we notified
the parents of the two that received citations. They received citations and were
released.

Officers were dispatched to 706 Cross St reference a standby. The caller advised
he had been locked out of his apartment by Nicole Phipps with his keys inside.
Phipps had an active felony warrant. Contact was made with Phipps and she
resisted arrest. Phipps also spat in the face of an officer during his official duties.

attracting the attention of subjects nearby. During the incident, Phipps made

SPALDING, S. L.

Suspect was refused service at Piston`s and had to be physically removed from the
bar by security. Officer Montgomery and Spalding requested the suspect leave the
festival and he refused. He was yelling and creating a disturbance with a large
crowd gathering. He continued to argue demanding he be let back in the bar
Officer Spalding told him he was under arrest and went to place a cuff on his left
arm. He pulled away and said he wasn`t going to jail. Officer Spalding grabbed
him and told him to get on the ground. Officers then had to physically pull the
suspect onto the ground. He continued to resist officers efforts to handcuff him.
A couple of citizens assisted in getting the suspect handcuffed. He`s refusal to
comply resulted in Officer Montgomery falling to the ground. She sustained
abrasions to her hand and elbow. It also resulted in her prescription glasses being
broken. Officer Spalding sustained abrasions to her hands.

When the victim returned she noticed that her 60" tv was missing. Upon further
investigation, the listed items were noticed to be missing. The TV in the child`s
bedroom was moved and may possible yield fingerprints.
The person made entry through the rear kitchen window. They cut the screen and
pried the window. They exited through the rear sliding glass door.

0249 and 0250 hours. While on scene with Guinn, a private number called Guinn.
Guinn had me answer the phone and then confirmed that it was Raider based on
his voice. Raider was making physical threats towards me while on the phone with
him. Raider eventually hung up.
82D01-1609-PO-004716 is the temporary Protective Order number which was
confirmed through records and served on Raider on 09/26/2016.

Ave, Evansville, IN, between 18:30, 10/06/2016 and 07:00, 10/07/2016. Reported:

Officers were dispatched to 3105 Taylor Ave for a residential burglary report.
The victims told officers that sometime between 1830 hrs on 10/06/2016 and 0700
hrs on 10/07/2016 their residence was burglarized. The victim found the rear door
forced open, the house ransacked, and multiple electronic items stolen. Also
stolen was a Ruger 10/22 rifle with a wood stock, a black pack that contained
miscellanies outdoor items, and video games. There are no suspects at this time.
CSU and K9 responded to the scene. The victim was given the case number of
the incident and given instructions to contact records with any additional items or
serial numbers of stolen items.

RP is an employee of ACE Home Improvements. Staff arrived this morning


and saw that tools were missing from one of the company trucks, which was also
partially spray-painted inside, and that the keys were missing from two company
trucks. The trucks are left unlocked and the keys are left inside the trucks. The
trucks are parked in an area behind the business which is partially
sectioned/closed-off, but not completely. So entry can be made without force.
RP has video surveillance of the incident. I advised him to save it until Crime
Scene contacts him. The driver of the truck with the missing tools had to leave in
the truck shortly after I arrived, so I was not able to get a list of the tools that were
taken. I advised RP to call Records and add the missing tools to the report at the
earliest convenience.

I was dispatched to 1 N Weinbach Ave Family Dollar on a shoplifter. Reporter
advised that the shoplifter/suspect was still on scene. When I arrived I spoke with
reporter who advised that the female suspect had left. Reporter said that she had
witnessed suspect place items into her purse. She said that she was waiting on
another customer at the register when suspect exited the store. She said that when
suspect left through the front door, the alarm sounded from the security tags on
merchandise. She said that she then walked out and stopped suspect. Suspect
followed her back into the store and she was able to retrieve the merchandise.
Reporter was given the case number to the report.

spoke with the offender using facetime on the victim`s phone. the offender stated
he gets the child on Fridays from 1200pm to Saturday til 12:00pm. Offender also
stated she is wanting money from him before allowing his visitations to be
exercised. I advised offender that she does not have the authority to designate a
child support number and would be up to the court to do so. Offender was advised
that a report will be generated if the visitation is not exercise. Offender again
denied his visitation and began to laugh.
Both, the offender and victim stated there is a court order for visitation only and
nothing in the order about child support. Victim was given a case number.

order more to drink. The reporter said she refused to serve her and asked her to
pay her bill. The female produced a credit card in a male`s name. Initially, the
reporter said she refused to try the card, but she later did try to run it. The card
came back declined. The female then kept saying her boyfriend was coming, but
no one showed to pay the bill. The female patron began asking other customers to
borrow money before leaving for a second time.

report. Officers arrived and met with the victim, the manager at Arrowhead Motel.
The victim informed officers he had been continuously receiving phone calls from
an individual who had been at the business earlier in the evening. According to the
victim, the customer was angry about being denied a room. Officers observed the
phone calls occurring while on scene with the victims.
Officers also spoke to another victim, an employee at the business. The victim
advised when the suspect was at the business earlier in the evening, he became
enraged and stated, "Since you`re not going to give my family a place to stay, I`ll
kill every one of you!" before leaving the business in a blue van.
The business did not have a caller identification system attached to their phones.

There appeared to be red paint transfer on the pole. There was no further
vehicle/suspect information at the time of this report. Vectren was contacted and

vehicle were minor scratches and dents to the rear driver`s side quarter panel. The
victim advised the offender of this and told her to wait, because she had already
called police. The offender refused and fled South bound on Green River Rd. As
the offender fled, she ran over the curb on Green River Rd and was all over the
road. Victim believes the offender was intoxicated. This occurred in the front
