Burden of disease

Executive Summary

The subnational burden of disease study
of Thailand by regional health and geographical
region in 2011 aimed to investigate the mortality
and disease burden in the Thai population, and
to prioritize health problems at regional health
and regional level for future policy support. The
study employed a cross-sectional descriptive
study approach. Data were obtained from
the Bureau of Registration Administration,
the Department of Provincial Administration, and
the Ministry of Interior Affairs. Mortality and
disability adjusted life years (DALYs) data acquired
from the Report of Burden of Diseases and Injuries
of Thai Population 2011 were analyzed by
regional health as described by the National
Health Security Office and the Ministry of Public
Health in 2013. The results of the study are
summarized as follows.

Mortality in Thai population by health

region and geographical region levels
in 2011

The standardized death rate was found the
highest in the northern male and female
population (861 and 659 per 100,000 people
respectively). At the health region level, the
highest adjusted mortality rate in males was found
in health regions 1 (889 per 100,000 people), while
females in health regions 7 had the highest

adjusted mortality rate (719 per 100,000 people).

Health regions 13 had the lowest rate both in
males and females (594 and 427 per 100,000
people respectively).

The standardized mortality ratio (SMR) was
highest in northern and northeastern parts of
Thailand including health regions 1, 2, 3, 7, 10.

Causes of death by disease group

and by disease

1) Cause of death by disease group:
cardiovascular diseases caused the highest
adjusted mortality rate both in the male and
female population in most of the geographical
regions except for the males in the northeast
where the main cause was cancer. At the
regional health level, cardiovascular diseases
caused the highest adjusted mortality rate in
males and females in most of the geographical
regions except for the male population in health
regions 7, 8, 10, and 13 whose residents
mainly died from cancer, and females in health
region 7 from diabetes.

2) Cause of death by disease: stroke had
the highest adjusted mortality rate in the male
and female population in most of the geographical regions except for males in the northeast
region whose major cause of death was liver
cancer and males in the south where ischemic

heart disease was the main cause. As for females

in the northeast, the main cause of death was
diabetes. At the health region level, cardiovascular diseases had the highest adjusted mortality
rate in most of the geographical regions except
for males in health regions 7, 8, and 10 who
mostly died from liver cancer and males in health
regions 11, 12, and 13 whose main cause of death
was ischemic heart disease. The majority of the
female population in health region 1 died from
chronic obstructive pulmonary disease (COPD),
whereas those in health regions 7, 8, and 10
mainly died from diabetes.

Burden of diseases in Thai population at health and geographical regional level 2011

The highest DALYs in males and females
were reported in the Northeast (757 and 581 per
1,000 people respectively). At the health region
level, region 1 has the highest DALYs in males
(251 per 1,000 people) while females in health
region 7 had the highest DALYs (167 per 1,000
people). Health region 13 had the lowest DALYs
both in male and female (136 and 104 per 1,000
people respectively).

in the male population in most of the
geographical regions except for the south
where cardiovascular diseases were the main

cause. Cardiovascular diseases were the main

cause of death for females in most of the regions
except for those in the northeast where diabetes
was the main cause. At the health region level,
injury was the main cause for the highest DALYs
in males in most of the regions except for health
regions 1, 7, and 9 where mental disorders were
the main cause of DALYs, and cancer for health
region 8. Females in most of the geographical
regions suffered from cardiovascular diseases
except for those in health regions 7 and 8 where
diabetes was the main cause, and depression was
the main cause of DALYs for females in health
region 13.

2) Burden of diseases by disease:
alcoholism contributed to the highest DALYs for
males in the north and the northeast, whereas
those living the central and the south regions
mainly suffered from road accidents. The female
population in most of the geographical regions
suffered from stroke except for those in the
northeast where diabetes was the leading cause.
At the health regions level, road accidents
contributed to the highest DALYs in males in most
health regions aside from the regions 1, 7, and 9
where alcoholism was the leading cause. Liver
cancer was the main cause of DALYs in health
region 8, and cardiovascular diseases for health
region 13. Stroke contributed to the highest
DALYs in the female population in most of the
geographical regions except for those living in
health regions 7, 8, and 10 who mainly suffered
from diabetes, and depression as the leading
cause in health region 13.
