We are happy to say that we pick your child ______________________________ as
the member of our volleyball team.
We, the Vallenian Volleyball players, would like to inform you that we are
going to conduct training your child that we pick for the varsity.
We would like to ask for your permission to allow your child in joining us as
well as your cooperation by signing this waver that whatever happens to your child
is not a part of the schools responsibility.
Players are required to wear sports attire. The said activity will be held at the
VHA Quadrangle 8:00 am in the morning to 12:00 pm at the afternoon in Saturday,
July 14, 2012
