“The Susick community is very diverse. Ithas many different ethnic groups and close to 50% of
our students speak a language at home other than English. We are one of the few schools in the
‘Warren Consolidated District that has such a diverse group of students. Our teachers are very
experienced and all are highly qualified in their field of study. Our staff and students are a family
and, asa family, we work well together and help each other out, when needed. Grade levels meet
‘on a regular basis to discuss upcoming plans and events, students” needs and ways to help the
students who are below grade level in their reading and mathematics skills.
‘MSs, Johnson is gaining a world of experience as we have 3 students who are on the autism
spectrum, Each of these students display behavior that Ms. Johnson has learned to effectively
handle to diffuse an explosive situation.

INTERPERSONAL RELATIONSHIPS
Ms, Kathy Johnson has become a valuable asset to our school community. She gets along well
With the students and the staff. She reaches out to help staff members with technology and any
other academic concerns that arise. Ms. Johnson is very skilled at reaching out to the students’
needs and has displayed the ability to help each student on a one to one basis. She comprehends
assessments given and helps inthe decision making process as to how to help each child in their

Ms. Johnson has excellent classroom management skills. She speaks clearly and lessons are
explained thoroughly. She has devised classroom posters of the skills being taught. The students
respond well tothe colorful displays. Ms, Johnson has become adept in noticing inappropriate
‘behavior before the student can fully exhibit their poor choice.
Every morning Ms, Johnson greets each child with a smile and a positive attitude toward learning.

daily work.

INSTRUCTIONAL PLANNING AND IMPLEMENTATION
Ms. Johnson spends a great deal of time after schoo! and on weekends planning for the
instructional day. The lessons are thorough and each standard is clearly stated. The time spent
lesson planning is evident when she presents the skills to the students. Ms. Johnson knows how 10
introduce a lesson, teach the appropriate skills, and she knows how to recap the skills taught.
INSTRUCTIONAL PLANNING AND INCORPORATION OF TECHNOLOGY TO ENHANCE
STUDENT LEARNING
Ms. Johnson has excellent technology skills. She has created lessons in Social Studies using a
Weebly account. The lessons are designed to help the students learn about American History in a
fun and informative way. Ms. Johnson also created digital storytelling whereby each student read
a section ftom a book and drew a picture based on what was being read. The students viewed the
digital storytelling and it helped them to visualize their fluency. Ms, Johnson also created a Prezi
lesson for instructing realistic fiction, It was well received by the students and it helped them to
understand the concepts being taught.
EVALUATION:
‘Ms. Johnson is very professional and is an active teacher. She gives totally to her class and to the
Susick community. She volunteers for afterschool activities. Ms, Johnson has built a positive
rapport with her students. She relates well with her students and she is constantly providing her
students with positive feedback. The lessons are well planned out and Ms. Johnson utilizes the
students” resources to support their learning. Specifically, each student shares their prior
knowledge of lessons taught.COMMAND OF SUBJECT MATTER
‘Ms. Johnson has excellent command of all subject matter taught in the fifth grade. She spends
countless hours after school and on weekends learning the material. She delivers the concepts in a
clear, concise manner and in a variety of ways to help each students learn. Ms. Johnson also is
adept in working with students one on one when they need extra assistance. She is impressive
with her knowledge and her ability to utilize all sources that our schoo! has to offer.
PERSONAL QUALITIES
Ms. Johnson has a wonderful personality. She is well received by students and by her peers. She
dresses well and speaks in a professional manner at all times. Ms. Johnson is friendly and helpful
to staff and students. She isa highly conscientious, well organized and caring. Ms. Johnson has
poise and displays self-confidence when working with students

‘Ms, Johnson is always professional in demeanor and when speaking to others. She is thorough
and always well prepared for the day. Ms. Johnson knows how to handle difficult student
