I would like to apologize for the late submission of documents for my Business Permit
Application of our Business in the name of AEVITERNITEA TEA SHOP located at P. Oliveros
St. Brgy. San Roque, Antipolo City in to your good office due to some reasons:
1. No Building Permit granted by City Engineering Office
2. The Building permit is still processing
3. The management will be preparing some legal documents regarding the business
operation
All of these problems was arises but I have to continue my business and to resume my renewal of
