JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS
We, ERIC P. GAMIT and EVERNESTO B. GALSIM JR., of legal ages,
Filipino citizens, and residents of Poblacion West, Santa Ignacia, Tarlac,
Philippines, after having been duly sworn to in accordance with law, do hereby
depose and state THAT:
1.

We both personally know GELYN BAGAYAS SAPINORO

(Gelyn, for brevity), born on May 18, 1983 to Spouses Alfonso Sapinoro
and Nely Bagayas in Camiling, Tarlac;

2.

Maureen is a member of the Social Security System of the

Philippines (SSS) with SS Number 02-1505479-9;

3.

However, the recorded name of Gelyn appearing in the

database of the SSS is GELYN CARBONEL SAPINORO, instead of
GELYN BAGAYAS SAPINORO as appearing in her Certificate of Live
Birth. Hence, the discrepancy;

4.

We are not related to Gelyn by affinity or consanguinity, except

that of being her barangay mates;

5.

We are executing this affidavit to attest to the fact that the

names GELYN BAGAYAS SAPINORO, appearing in her Certificate of Live
Birth, and GELYN CARBONEL SAPINORO, as recorded in her SSS
record, refer to one and the same person, and that is our barangay mate,
Gelyn;

6.

Finally, we are executing this Affidavit to attest to the truth of the

foregoing statements and for all legal intents and purposes it may serve.
