OCHANGE CONSENT FORM

Purpose of Research
This survey questionnaire is required for the completion of our major Organizational Change and Development
(OCHANGE) under Prof. Norman Zafra. This is to assess the organizations ability to communicate change with its
employees. This is also to asses the subordinates reaction and feeling toward how the new management helped
the employees to transition from the previous management to the current management.
Specific Procedures to be Used
In order to have a comprehensive understanding of the situation, the researchers will be conducting a survey that is
going to assess Road Boards employees perception of current change communication.
Duration of Participation
This survey questionnaire will only take 7 to 10 minutes of your time.
Confidentiality
No one has access to the information except the researchers and the professor. All data will only be used
specifically for this study and will be disposed one week after the term. You may choose to keep certain information
private such as your name. The use of pseudonyms is acceptable. However, your real information is still required
but will still be kept confidential. Throughout the duration of our study, no specific company information will be
recorded, most especially anything that could threaten the reputation of your company. What will be recorded are
actions, behavior, terms, and opinions, and reactions of employees interacting. Participation in this exercise will not
affect status/position/reputation in your company and no information recorded will go beyond the boundaries of your
department.
Voluntary Nature of Participation
You do not have to participate in this research exercise. If you do agree to participate you can withdraw your
participation at any time without penalty.
