Instructional Intervention Project Report Form

IEP Instructional Goal: Within 36 instructional weeks, when provided with a letter in written
format, student will verbally identify 20/26 letters in 4/5 charted observations.
Data Collection: Every Monday, Wednesday, and Friday, I will write down the number of letters
that the student gets correct and incorrect during a 10-minute period. (Ex:10/26)
Intervention: By using a white board and alphabet letter cards as a motivational tool when
writing letters, the student will be able to retain more letter names. Teacher will write letters and
have CS identify them verbally. If the student does not show improvement, the intervention will
be changed to include picture cards with the white board and alphabet cards.

After examining the graph, I see that the intervention was successful in helping the
student identify letters. When I first started the intervention, the student showed
improvement very slowly, but as I stuck with it, he began to show great improvement.
He began to retain more and more letters. When I added a visual, the student was
better able to retain the letter names. he began to show great improvement. He began
