Select one school-based program from the list and justify reasons of selecting this
program for this project.

PLANNING (5 marks)

Problem Statement (Why this program is needed to be carried out). Support your
argument with the literature in this area.

DESIGN OF THE PROGRAM (5 marks)

Who involve (describe in detail persons involved, the skills or expertise they have to
contribute in this program)
The duration of the program (describe in detail whether the program is one-off, shortterm, long-term)
The nature of the program (describe in detail whether it is a new program or adaptation of
an existing program)
Resources (budget and facilities)

IMPLEMENTATION OF THE PROGRAM (5 marks)

Describe in detail how the program is carried out. Support with appropriate documents used

Describe in detail how you evaluate the program. Justify reasons for choosing the methods.
Describe in detail the process of evaluation done, the output of the evaluation, implication of the
program and suggestions for future program.

You must support your explanation with appropriate references and appendix (handout, pictures)

ORGANIZATION, MECHANICS AND AESTHETICS OF THE REPORT (3 marks).

Use MS Words, font size 12 and double spacing. The length of the written project must be
between 20 25 pages (exclude the appendixes).
