Title of Artifact: Teacher Evaluation Form
Standard: Standard 5: Assessment- Teachers shall understand and use multiple assessment
strategies and interpret results to evaluate and promote student learning and to modify instruction
in order to foster the continuous development of students.
I am placing my Teacher Evaluation Form under Standard 5 Assessment, as evidence of
continued mastery. This form is able to let others know how I was as a student teacher and what I
need to improve on to better my skills and to perform to my highest ability when I become a
teacher. The evaluation will help me better myself and teacher other student teachers what to do
and what not to do.
