Dear Ms. Calloway,

Im writing to inform you that an incident has occurred on one of your filming locations. Last
week when my family and I were visiting this particular location, the set was open to visitors that
day, and an accident occurred. While we were standing roughly twenty feet behind the cameras,
away from production, an employee drove by at a very dangerous speed. Unfortunately, he was
operating the vehicle very close to the visitors, and unknowingly, running over my sons foot,
and injuring him.
He was taken to a local doctor and we later discovered that his foot had been indeed broken. He
is now going to be on crutches for at least a month. I believe that this accident occurred
unintentionally, and conflicts with your company policy. There was no employee on set who
informed us of a need for a signed waiver. Therefore, I wanted to inform you of what occurred. I
do hope that this matter can be settled in a simple manner.
In essence, I am writing you to request reimbursement for the medical bills that we received due
to this accident. After insurance coverage, the total sum should not be more than six hundred
dollars. To be clear, we do not want to pursue legal action, we simply request reimbursement. We
do hope that we can come to a swift agreement on this matter, and we look forward to hearing
from you. Thank you for your time.
