Platform felt down while reeving drilling line

Description
While performing the reeving of the drilling line, the travelling block which was laid down
on a platform got pulled towards the crownblock. The design of the platform was poor and
could not support the horizontal weight shifting applied due to the movement of the
travelling block. This caused the platform supports to break and fall.
A floorman was standing on the platform (crown block side) while the line was pulled by
the forklift. He lost equilibrium when the platform fell down and got bruised in the arm as a
result of rubbing against the drilling line.

Note: The platform was previously modified to include safety handrails & a fixed
access ladder
Lessons Learnt

A proper risk analysis is to be done before performing non-routine jobs, hazards must
be discussed and risk mitigated to minimize exposure.
Develop a JSA (Job Safety Analysis) sheet for this task (attached)
No person should be standing in that position (or in the way of possible moving
loads/shifting weights)
The travelling platform design is to be changed
