Dear Choir Parent,

This year, the 6th grade Choir has the incredible opportunity to perform at the Chicago Wolves
game on Sunday, March 5th with fellow choirs from Illinois. This performance will occur prior
to the game and all Choir members will receive a complimentary Chicago Wolves hat and ticket
to the game. Students who participate will be required to come to 4 rehearsals during their lunch
recess time on D days. This is not a required event, just a fun day to enjoy music and hockey!
Students must arrive by 2pm, the performance begins at 2:45pm and the game starts at 3pm.
Parent(s) or a designated adult must remain for the duration of the game and will be seated
near the Choir. Transportation to and from this event is not provided by the school. All
participants must provide their own transportation.
Discounted tickets are available to the families of each Choir member at a cost of $17.00 per
ticket.
At this time, I need to know whether or not your child will be participating in this opportunity
and if you would like to purchase an additional ticket for a parent/guardian. I am required to
submit the ticket orders on February 3rd to the Chicago Wolves.
Please read, fill out, and return the form below to Mr. Martinez no later than Friday, February 3
only if your child is going to participate.
