AFFIDAVIT OF PARENTAL CONSENT TO

MARRIAGE
I, Elisa Lorica Cristobal of legal age, Filipino citizen, married and
presently residing at #27 Pansol Ave., Barangay Pansol, Quezon City, after
having been duly sworn to in accordance with law, depose and state THAT:
1. I am the mother of Merwin Fiel L. Cristobal born in Quezon City on April 28,
1996;
2. My child is intending to contract marriage with Junezza C. Garay in Quezon
City;
3. I am giving my consent to our child, Merwin Fiel L. Cristobal, marrying said
Junezza C. Garay;
4. I am executing this Affidavit for the purpose of informing the authorities
concerned
of
the
veracity
of
the
foregoing
facts
and
for
whatever legal purposes it may serve.
