My present candidacy has been duly endorsed by a resolution of the Board of Directors
of my chapter, a copy of which resolution is hereto attached as Annex A and made
an integral part hereof ;

8. I have prepared a Plan of Action in the form prescribed by the JCP Secretariat or a
Financial Management Plan in the form prescribed by the JCP Commission on Audit, a
copy of which is hereto attached as Annex B and made an integral part hereof ;

I have fully and truthfully filled-up a questionnaire relative to my biographical data,

concepts and beliefs, a copy of which is hereto attached as Annex C and made an
integral part hereof ;

I have read and understood the provisions of the Constitution and By-Laws of the
Philippine Jaycees, Inc., the Policy Manual and the National Election Code of the
Philippine Jaycees, Inc. pertinent to and governing the qualifications of candidates, the
prohibited acts or activities during the campaign period and other similar matters;

11.

I possess all the qualifications and I have none of the disqualifications pertaining
to the office or position for which I file this Certificate of Candidacy ;

12. I voluntarily submit my candidacy to a consensus for national officers pursuant to

Sec.3, Art. VII of the Constitution and Policies Nos. 7.1 and 7.2 of the Philippine
Jaycees Policy Manual and that, in order to give effect to the will of the membership, I
do hereby irrevocably authorize the withdrawal of my nomination and/or candidacy in
the event that I do not obtain the requisite votes;
13. In case I am nominee representing an area or region, I hereby undertake the
mandatory obligation to obtain from the Area Comelec, a certification of my
election either as National Executive Vice President or National Vice President, as the
case may be, during the appropriate Area Conference, and to forthwith file the said
certification with the National Comelec, seeing to it that it is received therein at least
ten (10) days prior to the first day of the current National Convention, a copy of
which certificate shall be attached as AnnexD and made an integral part hereof;
14. I likewise undertake the mandatory obligation to obtain, at the proper time, a certification
by the Area Conference Director or the National Convention Director that I am
registered and in attendance at the Area Conference and National Convention at
which I am a candidate or nominee, and to forthwith file the said certification with
the National Comelec at the conference or convention site on the first day of the said
conference or convention , a copy of which certification shall be attached hereto as
Annex E and made an integral part hereof ;
15. I am remitting herewith the filling fee corresponding to the office or position for which I
