STATEMENT OF CPNI OPERATING PROCEDURES

1.
It is Moultrie Independent Telephone Company policy and practice that Customer
Propriety Network Information (CPNI) shall be treated as private, and protected from
disclosure at all times. The use of CPNI for any purpose not permitted by Section 64.2005 of the
FCCs rules or, except as may be expressly required by law, its disclosure to any other party for
any purpose is prohibited.
2.
All employees are aware of company CPNI policy and the obligation to protect the
confidentiality of CPNI.
3.
Access to CPNI data is limited to company personnel who need to use the information in
order to perform their jobs. All employees authorized to access CPNI data receive specific
training on the manner in which the confidentiality of CPNI data is to be protected and the
limited extent to which it may be used under company policy.
4.
Any employees improper use or disclosure of CPNI is subject to company disciplinary
policies.
