SELF-AFFIDAVIT
I, RIHAANA LIM, of legal age, widower, a resident of Ford St. Rosario
Homes, Dao, Pagadian City, Cebu City having been sworn to in accordance
with law do hereby depose and say:
1. I am legally married to late PIDDY DI having contracted marriage with
him on April 26, 1977;
2. That all the birth records of our children our DATE OF MARRIAGE were
indicated as APRIL 26, 1977;
3. Unfortunately however, when I secured a copy of our Certificate of
Marriage with the National Statistics Office I found our Date of Marriage
as indicated therein is APRIL 25, 1977 and not APRIL 26, 1977;
4. That the correct Date of Marriage is April 26, 1977 and not April 25,
1977
mistakenly indicated in the Certificate of Marriage.
5. That I am executing this Affidavit neither to attest to the truth of the
foregoing facts and not for the purpose of fraud nor to prejudice any
person or entity.
