The employment is subject to be policies of the Company as may be promulgated from time to time and the main terms and conditions are set below:~ LO ERTE ir (COMMENCEMENT DATE 1 August 2016 SALARY & ALLOWANCES Your basic salary shall be RM8,200.00 (Ringgit Malaysia: Hundred Only) gross per month. Your will receive a Site Allowance of RM300.00 per month Itwill be subjected to review solely at the absolute discretion of the company. (Please note that confidentiality of your salary should be maintained at all times. HOURS OF WORK Monday to Saturday: 08.00 a.m. - 05.00 p.m Lunch break 12.00 p.m, ~ 01.00 p.m. However, they are subject to change as and when deemed necessary by the Company. PROBATION You will be on probation for a period of Six (6) months subjected to extension of a further duration at the discretion of the Management. Thrcughout the probation period, your performance will be evaluated and on satisfactory completion, we will be pleased to confirin your appointment in writing, Page 1 of 3 raerey ere TERMINATION OF EMPLOYMENT a) Each party to this employment of Service may at any time give notice to the other party of their intention to terminate this Contract, b) The length of notice which shall be the same for either party is as follows: i) During probationary period — One (1) month notice or salary in lieu. ii) Upon confirmation of employment —Three (3) months’ notice or salary in lieu. ©) The Company reserves the right to terminate your employment at any time without prior notice or payment in lieu should you, in its opinion, commit one or more of the following:~ Dishonesty whether or not in connection with the employment, willful insubordination grave misconduct or non-compliance with the terms of this letter or any lawful orders or instructions given by the Company, incompetence or negligence in the performance of your duties, bankruptcy or failure to pay your debts within a reasonable time after they become due, conviction for any criminal offence, the taking of drugs prohibited by law, or the consumption of alcohol to a level which causes you to be unable to carty out the inherent requirements of your work ) tis an expressed condition of your employment with the Company that you engage yourself exclusively in the Company's business and do not undertake any other form of employment without the written consent of the Company. RETIREMENT AGE The retirement age shall be 60 years old. ABSENCE FROM WORK Unauthorized leave of absence from work for two consecutive working days shall render you liable to termination of employment. TRANSFER The Company shall have an absolute discretion to require you at any time and from time to time to work in any other office/site location and to carry out your duties with any subsidiary or associate companies. At the discretion of Management, you are liable to be transferred from one post to another within the Company or within the Group of Companies Page 20f3 I 9. MEDICAL AND PHYSICAL STATUS OF APPOINTEES Though you are not required to be medically examined, you are required to declare your fitness for employment and further required to disclose any illness, medical or physical disability which you have suffered or which you are suffering from which would affect your fitness for employment. Should it be found after employment that you have misrepresented or not disclosed, interlaid, material facts regarding any illness, medical or physical disability, it shall be deemed to be sufficient cause for the Company to terminate your service without notice and the Company shall not be liable to any claims for benefits thereafter. 10. CONFIDENTIAL & PROPRIETARY INFORMATION As a condition of employment, you shall keep secret and shall not reveal or disclose to anyone outside the Company without the prior written consent of the Company, all confidential and proprietary information relating to the business of the Company which come to your knowledge as a result of your employment by the Company. You shall not make use of such confidential and proprietary information for your own purpose or for the benefit of anyone other than the Company. Excess of leave than your annual leave entitlement shall be utilzed not later than March in the next calendar year. 50% of balance leave rnot been utilized after March will be forfeited. Another 50% of balance leave will be paid as leave in liou fo your April Salary.) Sick Leave ‘On recommendation of a registered medical practitioner, you will be entitled to paid sick leave not exceeding in the aggregate of - > Less than 2 years ~ 14 days > More than 2 years ~ 18 days > More than 5 years ~ 22 days Hospitalization Leave ~ 60 days including outpatient leave in the aggregate per calendar year. Medical Claims Out-patient medical claims is max RM2,000.00 per year. You are not entitled to claim Dental and Optical expenses. Out-patient does not include annual medical check-up unless recommended by doctor for diagnostic purpose. EPF/Socso Contribution Contribution to this fund shall be in accordance to the Employees Provident Fund Act 1991 (Act 452) & Regulations and Rules. Employer: 14% - 16% (subject to category of Designation) Maternity Leave 60 days up to the 5th confinement Paternity Leave 2 consecutive working days Marriage Leave 2 consecutive working days Compassionate Leave 2 consecutive working days with full pay in the event of death of employee's parent, Parent in-law, siblings, spouse or children, Legal Guardian.
