Provident Benefits with the Fund;

the applicants birth certificate, however, no record can be obtained,
as shown by the attached Certification of Non-Availability of Birth
Record from the National Statistics Office (NSO);

4. That we certify that he/she was born at ___________________,

5. That he/she has been using the said birth date in all his/her
documents and transactions;

6. That we are not related by affinity or consanguinity to the above

mentioned person; and
