ARTIFACT NAME: Transition Planning Document (IEP)

Rationale Statement:

The Transitional Planning Document represents my experience writing an appropriate

transition plan based off of a students individual need. In the case study we learned that Lisette

was preparing for her transition process and she will soon be a high school graduate. The

transition plan highlights her wants and desires for her future as well as any steps that need to be

taken in order to achieve her goals. The writing experience required me to identify members of a

transition team, any supporting organizations, and their individual responsibilities. Also included

in this plan are the techniques needed to perform a smooth and efficient transition.

This artifact has been included in this standard because it required me to take into

consideration her individual needs as well as her level of education. I was forced to think about

the obstacles she might face post-graduation in comparison to students in her same class. The

time spent discussing Lisettes learner differences allowed me to create a transition plan that is

well rounded and appropriate to her needs. I believe that I have created a well-executed

transition-planning document because it shows that I have the ability to set realistic and

achievable goals for students.
