explosion, the causes of the Challenger explosion and how it affected NASA and

future launches. The Space Shuttle Challenger explosion occurred on January 28, 1986. No
one inside the shuttle survived. The astronauts on the space shuttle were Francis Scobee, Mike
Smith, Judith Resnik, Ellison Onizuka, Ronald Mcnair, Gregory Jarvis, and Christa McAuliffe-
the teacher. Against the engineer's recommendations Thiokol and NASA decided to launch the

space shuttle Challenger . On January 28, the crew boarded the shuttle and lifted off from the
Kennedy Space Center.
Rocket fuel leaked from the O-rings in the SRBs resulting in the craft exploding about a
minute after lift off. With the nation in shock, NASA, Coast Guard and Navy ships retrieved
wreckage of the craft from the ocean. An investigation took place and it was found that the
O-rings were the cause. NASA and Thiokol worked together to redesign the SRBs and
launched another space shuttle, Discovery, two and a half years later. In brief, the book Fatal
