BACKGROUND OF THE STUDY

This study was conducted at Barangay San Miguel Bukidnon, at P-1A. The purpose of

this study is to assess the family, identify problems, plan and intervene through well- organized

nursing actions and perform necessary interventions, evaluate the outcomes and to gather data to

have a conclusion pertaining to the problems identified that serves as the result of the study.

The researches were assigned to the family of Mr. and Mrs.Manangkila. Comprehensive

assessments were made from the initial assessment and conducted on-going assessments

performed on the length of four months.

The researches chose the family for the reason that this family has a lot of problems that

concern the health care team, like malnourished children, poor environmental sanitation and the

like. This family is also the concerns of their Barangay Health Worker in their community.

The researchers were able to conduct assessments about the familys current condition

and how it could affect their health and wellness, family health problems were then identified as

wellness state and health threat. Necessary nursing actions were then performed during the time

allotted for the case study and the same provisions of health teachings.
