INCIDENT

Description of Incident:
On the above mentioned date, a chemical substances was spilled in endoscopy ward at the
ground floor of building. One of the staff accidentally spilt the substance when working on to
decontaminate clinical equipment used in the endoscopy department. This potentially
dangerous substances had spread quickly in the room with contaminated gas and forcing the
patient, visitor and staff being evacuated from the hospital. Fire and rescue services had taken
out the patient while inpatient stay on wards. Emergency continued, although there was slight
delay in some operations and thankfully no one was injured.

ACTION TAKEN:
1) Fire services responded and taken measures to ensure safety of the container storing the
substances
2) The substances was removed to a secure part of the hospital away from the main building
3) Further measure were taken involving specialist agency to ensure that its can be safely
disposed
4) The room where the spill happened was ventilated.
