3s 32. 33. 34. x. 5794 Regular Meeting 22 June 2016 CONTRACTED SERVICES AGREEMENT 2016-2017 SCHOOL YEAR Upon a motion by Marra, seconded by Miller, the request for approval to enter into an agreement with THERAPY ASSOCIATES, LLC and the Lodi Board of Education in order to provide services for #01-03 for the 2016-2017 school year is hereby approved unanimously. Services will include: special education 5 hours/week; physical therapy 3x45 min/week; services may also include speech therapy 2x/45 min/week and occupational therapy 2x/45 min/week. At the following rates: Said agreement is effective July 1, 2016 through June 30, 2017, a. OT, SLP, PT practitioners per 45 minute sessions: $120.00 b. Special Education teacher per hour session: $100.00 Preparation for, and attendance at IEP meetings shall be billed at same rate as therapy sessions at a pro rata basis. LEGAL SERVICES AGREEMENT Upon a motion by Marra, seconded by Miller, the request for authorization to enter into an agreement with the law firm of Sciarrillo, Cornell, Merlino, Mc Keever and Osborne, LLC in order to serve as Labor Counsel/Negotiator and Special Counsel for the Lodi School District from July 1, 2016 to June 30, 2017 at a rate of $160.00 per hour is hereby approved unanimously. STAFF APPOINTMENTS Upon a motion by Marra, seconded by Miller, the request for authorization to fill any staff vacancies during the months of July and August and have the Board confirm same at its next meeting is hereby unanimously.
