175.1 N. Gonorrhea Disseminated gonococcal infection. Neisseria gonorrhoeae
infection typically produces urethral discharge and dysuria.1 However, 1 to 3
percent of patients with gonorrhea develop disseminated infection and present
with one of two forms.2
The first form is an arthritis-dermatitis syndrome consisting of tenosynovitis,
dermatitis, and polyarthralgias without purulent arthritis.2 Acute illness is
characterized by fever, chills, and malaise. The tenosynovitis often involves
multiple tendons. Skin lesions (three to 20 in number) include painful
erythematous macules less than 1 cm in diameter that evolve to hemorrhagic
pustules within 24 to 48 hours and are found near small joints of the hands and
feet. The pustular vasculitis, intravascular thrombi, arthralgias, and tenosynovitis
result from hematogenous dissemination of the gonococcus.
The second form is a purulent arthritis without associated skin lesions. Most
patients are afebrile and present with an asymmetric polyarthritis. These
strikingly different clinical presentations may overlap, with the dermatitis
arthritis syndrome progressing to purulent arthritis.2,4 In rare septicemic forms,
disseminated infection may lead to hepatitis, meningitis, endocarditis, or other
systemic involvement.

Most cases of pelvic actinomycosis have been recorded in women who were
using an intrauterine device (IUD) form of contraception.
An IUD is a small, T-shaped contraceptive device made from plastic and copper
that fits inside the womb. The women affected by the condition tend to be long-
term users of an IUD (eight years or more).
