IEP TEAM CHAIRPERSONS: READ the following, bulleted information to the parentiguardian:
* _ Parentiguardian of the student was provided with the Parental Rights/Procedural Safeguards
notice and reminded of the right to appeal decision(s) of the IEP team to the Supervisor of
Special Education at (301) 644-5295. (Check as appropriate.)

(1 Parent/guardian declined copy and initialed previous receipt.
2 Parent/guardian did not attend, Copy of Parental Rights/Procedural Safeguards notice and
IEP team meeting notes will be sent to parent/guardian.
+ Farent/guardian of the student was informed of their right to mediation if they disagree with
decisions made at the IEP team meeting. Mediation is described on pg. 18 of the Parental
Rights/Procedural Safeguards notice.
Documentation of discussion: Introductions were made and the purpose of the IEP was
shared. A phone call was made to Ms. Halll and she shared that she and Mr. Giles will not
be able to attend and requested that the meeting proceed and paperwork be sent home.
Habilitative Services and Parental Rights documents will be sent home with the paperwork.
Ms. Smith shared that Jamal is a very sweet student who has a difficult time carrying on a
Conversation with peers and adults. He is reading at the B level which is a Kindergarten
level. Jamal can read 19/40 K sight words. He is working in a small group setting in math
working on first grade skills. He is working in the Math Navigator program. Touch math
methods were used in third grade with some success to add single digit numbers up to
five. Jamal has no appropriate peer interactions with his fourth grade peers, Jamal is
accessing no grade level skills at this time, even with modifications and accommodations,
Jamal needs adults support to accomplish all tasks.
Ms Householder shared that Jamal has been instructed in many programs: Reading
Mastery, Failure Free Reading, SIPPS, Sound Partners, LLI in Kindergarten and he
