CDP TRAINING GUIDELINES

1.0 CONDUCT OF TRAINING


1.1 Training hours
1.1.1 Lecture schedule is from 9am 6pm, Monday - Friday
1.1.2 Breaks in between sessions as needed, lunch break is one (1) hour
1.1.3 Clinical Skills Laboratory is a simulated clinical venue; provisions for 8-
hour shifting should be expected.
1.1.4 Clinical attachment schedule will follow the scheduled duty hours of the
preceptor assigned to the trainee; Saturday shifts should likewise be
anticipated.
1.1.4.1 Trainees are required to render at least eight (8) or ten (10)
hours per day depending on clinic schedule, with a total of
forty (40) hours per week.
1.2 All training programs shall be completed as per schedule.
1.2.1 Regular and/or Special non-working holidays are already considered in
the lecture week.
1.2.2 Clinic duty continues even during regular and/or special holidays.
1.3 Changes in the schedule should be anticipated on the following conditions
only:
1.3.1 "Force Majeure" - severe weather conditions
1.3.2 Unforeseen technical and clinical placement problems that significantly
compromises the trainee's learning outcome.
1.4 The training program welcomes constructive feedback from trainees as a
quality initiative.

2.0 CODE OF CONDUCT


2.1 Scope of Function
2.1.1 The trainee-preceptor booklet contains prescribed activities that
constitute the trainees scope of work and responsibilities; the
preceptor and the trainee are expected to adhere to these guidelines
accordingly
2.1.1.1 The trainee-preceptor booklet shall guide both the preceptor
and trainee in ensuring that they meet the learning objectives
and monitor their progress during the clinical exposure.
2.1.1.2 The trainee MUST, at ALL times, be supervised by a preceptor
whilst performing the activities set in the trainee-preceptor
booklet.
2.2 Observe professional decorum at all times.
2.2.1 Adhere to House Rules in the classroom (Annex B).
2.2.2 Adhere to SOPs in the clinical skills lab and in the dialysis clinic. This will
be discussed separately during orientation.
2.3 Patient confidentiality must be maintained at all times.
2.3.1 Patient information must be discussed or shared only with healthcare
providers directly involved in the patients care management.

2.3.2 Responsible and discretionary use of social media platforms
2.4 Adhere to the dress code as follows:
2.4.1 Lecture week Business Casual/Smart casual
2.4.1.1 No wearing of jeans (denim), short skirt, short pants,
sleeveless or backless blouse
2.4.1.2 No wearing of sneakers, slippers, open flat sandals
2.4.2 Clinical Skills Laboratory and clinical placement duty
2.4.2.1 Prescribed scrub suit and white rubber shoes and NO slip-ons.
2.4.2.2 Trainees are required to wear street clothes when traveling to
and from the clinic. Clinic uniform must only be worn while on
duty for infection prevention and control measures. Violation
is subject to disciplinary actions stated in item 5.0
2.5 All trainees shall observe proper grooming especially in the clinical setting.
2.6 All trainees shall be accountable as a group for proper use and care of
equipment during practice in the Fresenius Medical Care skills laboratory.

3.0 TRAINING EVALUATION


3.1 Written examination passing mark is 85%.
3.1.1 Failure to reach the passing mark of 85%, re-sit will be given.
3.1.1.1 If the trainee is still unsuccessful in passing the exam re-sit
he/she shall be subject to the provisions on Disciplinary action

3.2 Trainees are assessed based on simulated skills performance during Clinical
Skills Laboratory (2nd week).
3.2.1 The skills assessment is deemed either PASS or FAILED.
3.2.2 If the trainee demonstrates FAILED marking in any of the skills criteria,
the lecturer should identify and re-assess.
3.2.2.1 If the trainee Failed in skills re-assessment he/she shall be
subject to the provisions on Disciplinary action 6.3.
3.3 Evaluation Day
All duly accomplished training documents shall be forwarded to the CDP Admin
on the last day of training (Evaluation day) for timely release of training
certificates.
3.3.1 Trainee / Preceptor booklet

3.3.3 Time cards (attached Letter of absence and change of schedule form, if
any)
3.3.4 Course evaluation feedback form
3.3.5 Preceptor Evaluation Feedback form

4.0 ATTENDANCE POLICY
4.1 Absences
4.1.1 All notification for absences shall be made to the lecturer/preceptor at
least two (2) hours before the start of the shift. Notifications through
text messages are considered official only when given a confirmation
through reply by the CDP coordinator/Admin/preceptors. Likewise,
notification given or sent to anyone else/colleagues is not acceptable.
Failure to notify such absence will be considered unauthorized and/or
AWOL (Absence Without Official Leave).
4.1.2 Relevant document/s to support request for leave shall be submitted to
the lecturer/preceptor upon reporting back for training.
4.1.3 Approval for absence, aside from medical conditions, is the sole
discretion of the preceptor and the CDP Coordinator.
4.1.3.1 Approved absences accumulating to 16 training hours maybe
accorded make up duty privileges. (See 4.0)
4.1.3.2 One (1) occurrence of absence without official leave or
approval (AWOL) from instructor/preceptor is a ground for
dismissal from the training program.
4.1.4 Absence due to medical condition require submission of Medical
Certificate; approval is subject to the following conditions
4.1.4.1 No previous request for change of schedule of the same date/s
as the sick day/s
4.1.4.2 Medical certificate should be dated on the day of the absence
4.1.4.3 License number and contact details of the MD are evident in
the medical certificate.
4.2 Request for change of schedule must be accompanied by a properly filled out
Change of Schedule Form (Annex A) signed and approved by the
preceptor/Unit head/CDP coordinator, subject to the following conditions:
4.2.1 Another RN trainee has agreed to exchange shift
4.2.2 His/her clinical preceptor is available to supervise the RN trainee on
new shift
4.2.3 Request on change of schedule should be made by the trainee 5 days
before the scheduled date (unless emergency, less than 5 days of
notification may be considered). Otherwise, it will be considered
disapproved.
4.2.4 This form shall be attached to time sheet and submitted to CDP Admin
on the last day of training.
4.3 Tardiness
4.3.1 Trainees are expected to report to their respective clinics at least 30
minutes before their shift starts.
4.3.2 Trainee must notify his/her preceptor if he/she will not be able to
report for shift on time at least two (2) hours before the start of the
shift or in emergency situation. Notification through text messages are
considered official only when given a confirmation through reply by the

CDP coordinator/Admin/preceptors. Likewise, notification given or sent
to anyone else/colleagues is not acceptable.
4.3.2.1 Tardiness accumulating to more than 45 minutes AND / OR
three (3) occurrence of tardiness, whichever comes first,
during the entire training period shall be considered as
habitual and shall be subject to the provisions on Disciplinary
Actions.
4.3.2.2 In addition, every 45 minutes cumulative late will be equated
to an eight (8) hour-deduction from the total training hours
rendered.
4.4 Under time
4.4.1 Trainee must complete forty (40) hours per week.
4.4.2 In case a trainee would like to leave earlier than the end of scheduled
shift, he/she should seek prior approval from his/her preceptor.
4.4.2.1 Any minutes/hours of duty not completely served by a trainee
shall be reported to CDP Admin and will result to deduction
from total contact hours.
4.4.2.2 A total of 3 occurrence of approved undertime, regardless of
number of minutes, for the whole training period, he/she shall
be to the provisions on Disciplinary actions
4.4.2.3 However, if a trainee logs-out without approval from the
preceptors/CDP coordinator before completing the duty
schedule, deductions will be applied and will subject for
suspension.
5.0 SCHEDULING OF MAKE-UP DUTY
5.1 Make-up duty for absence with valid reasons may be scheduled ONLY IF ALL of
the following conditions are met.
5.1.1 Approval of absence from the lecturer or clinic Team Leader.
5.1.2 Valid and complete medical certificate was presented to the clinic Unit
Head or CDP Admin upon return. (See 3.1.4)
5.1.3 Approved change of schedule by clinic Unit Head. (See 3.2)
5.2 Make-up duty shall be scheduled on the trainees day-off and training days
cannot be extended.
5.3 Failure of trainee to comply with any one (1) of the condition above shall
render any make-up duty as violation of prescribed process. Hours rendered
will not be counted.

6.0 DISCIPLINARY ACTIONS


6.1 Written Warning
6.1.1 To be done immediately by the lecturer/preceptor when there are
observed performance inadequacies or inappropriate behavior as
outlined in this guideline.
6.1.2 The trainee will be required to submit a letter of explanation stating
his/her own corrective action.

6.1.3 Proper documentation will be made in the Trainee-Preceptor Booklet
accordingly.
6.2 Preventive Suspension
6.2.1 Receipt of at least 2 reprimands or warnings and/or at least one (1)
written warning on conditions stated above.
6.2.2 Violation of procedures in scheduling of make-up duty (Item 4.0)
6.2.3 Poor Attendance
6.2.3.1 Absences, as approved, but accumulating to more than 16
training hours.
6.2.3.2 Habitual tardiness (Tardiness 3.3.2.1)
6.2.3.3 Undertime (4.4.2.2)
6.2.4 Involvement in an adverse alert, incident report or clinical variance
while pending investigation and resolution.
6.2.5 Performing clinical activities not included in daily activities presented in
CDP booklet. Ex. assumption of staff RN functions
6.2.6 Proper documentation will be made in the Trainee-Preceptor Booklet
accordingly.

6.3 Dismissal or Discharge from the training program.


The following are grounds for dismissal or discharge from the training program.
6.3.1 Failure to reach the passing mark of 85% in any evaluation phase of the
training Theory (written exam) and Clinical skills laboratory.
6.3.2 Willful disregard of training guidelines as well as clinic policies and
procedures that resulted to an Incident Report or Clinical Variance
Report where negligence, commission or omission has been
established.
6.3.3 Grave misconduct resulting to damage of facilities and/or equipment in
the clinical skills laboratory and the clinic.
6.3.3.1 Trainees shall likewise be held accountable and liable for any
damage, and accompanying repair costs, resulting from misuse
and mishandling of equipment in the clinical skills laboratory
and Nephrocare Clinics.
6.3.4 One (1) occurrence of absence without approval from
instructor/preceptor.
6.3.5 At least one (1) occasion of suspension maybe considered as ground for
dismissal if issues on performance or behavior persists.

7.0 ISSUANCE OF TRAINING CERTIFICATE
7.1 Trainee will be given a certificate to signify extent of accomplishment in the
training program
7.1.1 Certificate of Completion shall be given, provided the trainee has
successfully met the following criteria:
7.1.1.1 With a passing grade of 85% in any evaluation phase of the
training:
7.1.1.1.1 Final Written Exam
7.1.1.1.2 Clinical Skills Assessment by the preceptors
7.1.1.2 Demonstrated good conduct for the duration of the training
Completion of 240 hours training (40 hours lecture, 200 hours clinical
including skills laboratory). OR
7.1.2 Certificate of Attendance indicating total training hours rendered with
percentage mark maybe granted on the following conditions
7.1.2.1 Voluntary withdrawal from the training
7.1.2.2 When a trainee was subjected to disciplinary action, Fresenius
Medical Care reserves the right to determine the hours that
may be credited to the trainee depending on the gravity of the
infraction.
7.2 Training certifcates are released on the last day of training if training
documents are submitted on time (see 3.3) and no clinical variance report
for investigation.
7.3 Lost training certificate will not be replaced but letter confirmation of
training will be provided.

ANNEX B

HOUSE RULES

1. Discuss all concerns in a PROFESSIONAL AND COURTEOUS manner.

2. The Admin Office is open from 8:00am 12:00noon and 1:00pm 5:00pm Monday to
Friday. Please transact with the office during business hours only.

3. Training and reference materials are given in advance. Please do advance reading and come
to class prepared for discussion.

4. Training IDs must be worn at all times within the premises of Fresenius Medical Care Head
Office or in the clinical venues.

5. Laptops or tablets may be used in the classroom only as a learning tool. Internet browsing
while there is ongoing lecture is strictly prohibited.

6. Turn-off your mobile phones or put it in silent mode while in class. If you need to answer an
urgent call, please step outside the room to take the call.

7. Modulate your voice as a courtesy to others, especially if there are simultaneous trainings in
the facility.

8. Take care of your personal belongings; FMC will not be liable for any loss. In the event of
loss, the security guard will assist in the investigation.

9. Food and drinks are not allowed to be consumed inside the training rooms. Staff lounge is
available for use during scheduled break time.

10. Discard all waste materials in trash bins properly. All types of tissue papers must be thrown
into designated bins only.
11. Please discuss any relevant training issues directly with the CDP Admin, CDP Coordinator,
Preceptor or Clinical Team Leader as appropriate. Do not let others speak for your concern
to minimize misunderstanding.
12. Please ask the assistance of security guard if you have official business with staff in second
floor office and clinic facilities.
