ISSUE(S) TO BE ADDRESSED:
- Lack of work/refusal to work / can be pleasant / will not take pencil / wanders around classroom
- Walks out of the room / makes a big production out of everything he does
- Withdrawn not appart of school life / lacks maturity and social skills of a high school student. Very
impulsive and doesnt have friends and seeks them via poor behavior but in soe classes he does appear to
have friends who will sit with him and interact.
- Mild defiance makes it clear he doesnt want to be there / escape his classrooms
- Doesnt produce a lot of work/ hides his difficulties by task avoidance
Student Strengths STUDENT NEEDS
Personable one on one / sweet kid when no Learn boundaries / social circle of friends to
expectations belong with his peers
Wants to give what he has to others / good cook Involved in something at school that focuses on his
strengths / needs to follow the classroom activity -
routines
Has the ability to be successful in art / doesnt seem He needs to do some work. This year he needs to
to get angry be more independent and self-directed.
He is eager to help / very hands on and likes to fix He needs a mom. Wants to feel like people like him
things and he needs a lot of support / place to go at lunch
time
WHAT STRATEGIES HAS THE REFERRING TEACHER ALREADY TRIED?:
Contacting home with very limited success. Tried to sit with him one on one but the more attention
provided the less he produced more interested in social conversation with adult than the work.
Reducing assignment load. Noon detentions for failing to do work. Sits near teacher but not at a desk.
Sits on a stool or stands to work. Do this first and then produce then reward. Have him working with
other students. He produces verbally others record for him.
He keeps all of his things in the classroom therefore he doesnt need to leave to get things. Tried this in
the front of the room tried in the back
Tried to interact with him positively at the beginning of class. Tried to remove him. Tried to provide him
with supplies quietly. Tried to place him with a variety of students to support him. Provide him with
work he can do based on his pre-tests.
